1) A winner is selected at random each month to receive free laser eye treatment up to the value of £3570, under legal supervision.
2) Transferable treatment prizes of £1000 are also available.
3) Treatment is subject to a medical consultation, and the surgeon’s decision on suitability is final.
4) Please note that 25% of those attending consultation prove unsuitable.
5) All prizes are transferable to other laser treatments.
6) A small deposit is required to secure an appointment.
7) Winners must be aged 21 or over.
8) No cash alternative is available.
9) By entering draw you agree to receive information from Optimax.
